Procedure - Fracture repair - radius | Performed by - AD | Surgery time - 60 minutes | Pre-medicated with torb IM. Anesthestized via isoflurane facemask,
then intubated with 4.0 uncuffed ET tube. Plucked and scrubbed
dorsal and ventral aspects of the patagium, plucked and scrubbed
dorsal aspect of the radius. Cleaned and debrided edges on the
ventral aspect of the patagial wound, closed skin with 4-0 webcryl
in horizontal mattress sutures. Then cleaned and debrided the
dorsal patagiial wound and repaired in the same manner. Incised
over the dorsal aspect of the radial fracture site, dissected
through muscle and fibrous callus to fracture ends. Placed IM
pin in the distal fragment, drove out the bent carpus and then
drove across the fracture site to seat in the proxiaml fragment.
Closed muscle and skinover fracture site with 4-0 webcryl in
simple interrupted sutures. Placed TAO, telfa/tegaderm bandages
over the sutures on both sides of the wing, placed tegaderm over

O: Checked vitals. Performed intake exam. Noted R patagium wound and radius fracture. Gave torb. Took rads. Cleaned patagium wound w/ chlorohex. AD assessed patagium wound. Applied Duoderm gel covered wounds w/ telfa/tegaderm. Applied paper tape around R wing. Gave fluids and oral meds. Placed in cage in ICU.

Wings: Large R patagial wound. R proximal radius fracture. Legs: NSF Feet: NSF Body: NSF Feather condition: In blood Radiographic findings: R proximal radius fracture, slightly misaligned. Initial treatment: 0.23cc Torb; 0.39cc Meloxicam; 1.16cc TMS
A: Baby with large skin tear over R patagium. Scabbing present over ventral aspect of wound that is also over some muscle on R ulna. R radius fracture also present under patagial wound. AD assessed that baby will need surgery as soon as possible to debride patagium wound and potential place radial pin. Baby is alert and vitals are stable enough for surgical repair later today.
P: Rx Meloxicam, TMS, fluids. HF BID in camo. Surgery today.
